Registration and What to Bring (Web Registration Form)
We ask that patients please arrive 15-20 minutes prior to your appointment so that the front desk may collect your information. Please be sure to visit our Web Registration Site to update your demographic, insurance and medical history information. Please be sure to bring your insurance cards to every appointment and notify our front desk that you updated your information on our registration page. We also request that all patients bring their current pair of eyeglasses, a list of current medications they are taking and bring any bottles of eye medications with them to each appointment. In addition, if you are a contact lens wearer and want us to evaluate your contact lenses and renew your prescription, please wear your contacts in for your appointment and bring your glasses. If you are a NEW patient that wears contact lenses you must bring your contact lens boxes or parameters with you to your appointment if you would like us to evaluate your lenses. Please allow an hour and a half for your appointment. Your time in our office may be longer, depending on your specific needs.
Dilation
In order to perform a comprehensive examination, dilating drops may be used. Dilation may temporally blur your near vision as well as make you light sensitive. You may want to bring someone to drive you home.
Minors
All patients under 18 years of age must be accompanied by a parent or legal guardian and are not permitted to drive while dilated.
Contact Lenses
If you currently wear contact lenses, please inform our staff at the time your appointment is scheduled. Contact lens evaluation and/or fitting is not included in an annual comprehensive or routine exam and may require additional visits. Please inform our staff if you are interested in contact lens fitting/refitting or renewing your contact lens prescription when you schedule your appointment. If you are a NEW patient, you must bring your contact lens boxes or parameters with you to your appointment if you would like us to evaluate your lenses.
Emergencies
If you are experiencing an urgent eye problem, please do not hesitate to contact us. Our Patient Coordinators are available by phone Monday, Wednesday and Friday from 8:30 am – 4:00 pm and Tuesday and Thursday from 8:00 am to 3:00 pm. We do allow time in our Doctors schedules for patients experiencing medical eye emergencies.
24 Hour Cancellation & “No Show” Fee Policy
Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, Frederick J. Sauerburger, M.D., p.a. reserves the right to charge a fee of $50.00 for all missed appointments (“no shows”) and appointments which, absent a compelling reason, are not cancelled with a 24-hour advance notice.
“No Show” fees will be billed to the patient. This fee is not covered by insurance and must be paid prior to your next appointment. Multiple “no shows” in any 12-month period may result in termination from our practice.
